What the inspector wrote

On July 11, 2025 at approximately 9:00am, Licensing Program Analysts (LPAs) Catrina Quimbo, Leslit Tapia Mandujano, and Centralized Application Bureau (CAB) LPA Andrea Medlin, conducted a scheduled, pre-licensing visit, to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. LPAs met with applicant, Grace Yee and explained the purpose of the visit. Also present during visit included head teacher, Wanying Deng. This is a relocation application. Applicant is currently licensed for 414003092. Applicant is aware currently licensed facility will close when relocation application has been approved. Applicant is requesting to be licensed for a total capacity of 60 preschool children (ages 2 years to entry into kindergarten). The fire clearance was approved on July 8, 2025 for 60 preschool children. The center will operate Monday through Friday 8:00am to 6:00pm. LPAs toured and measured the on-limits areas and classrooms located at the facility. The facility includes a director's office (with an isolation restroom), teacher/food preparation area, and outdoor teacher patio. The facility has a total of 3 preschool classrooms: Strawberry (ages 3-3.5 years old), Blueberry (ages 2-3 years old), and Pineapple (ages 3 years to transitional kindergarten). Preschool Classroom measurements are : The Indoor total measurements contain a total of 2,226.76 square feet which will accommodate Applicant\342\200\231s request for 60 preschool children. Children will have access to 5 sinks and 4 toilets which will accommodate applicant's request for 60 children. Preschool Outdoor Play measurements are: Outdoor total measurements contain a total of 5,164.75 square feet, which will accommodate Applicant\342\200\231s request for 60 preschool children. Based on measurements taken today, the requested capacity will accommodate the 60 children. (Continue Report on Page 2...) (Continued, Page 2...) LPAs observed all indoor activity space to be complete with safe, age appropriate furniture, including tables, chairs, napping cots, bookshelves and other activity supplies for the children. LPAs informed Grace Yee that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, applicant stated that they will be in a high cabinet located in teacher/food preparation area, inaccessible to children. There is a fully equipped first aid kit on site. Per applicant, detectors on site are dual smoke and carbon monoxide detectors. LPAs advised applicant to install a separate carbon monoxide detector that can be tested during a future visit. All required licensing documents were observed to be posted in the main entrance to facility. Children will be signed in and out of program with electronic signatures. Per applicant, each authorized representative has their own individual code that is specific for the individual. Applicant plans for each classroom to have a separate entrance and exit that families will have access to. Children's restroom was observed to be in working condition. Sinks accessible to children do not provide water that is warmer than 120 degrees Fahrenheit. Changing table has at least 1" padding covered with moisture resistant material. Sides of the changing table are at least 3" high and the changing table is within arm's reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Program plans to utilize cots for napping children that will be stored in the classrooms. LPAs observed cots to be labeled with children's individual names. Each cot shall be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child. Applicant plans for children to bring their own sheets and blankets that will be sent home weekly for laundering. Sheets will be stored in children's individual cubbies, to avoid touching each other. Per applicant, cots are sanitized daily. Drinking water is available with children's individual water bottles that will be labeled with their individual names. Children's water bottles can be refilled in on site with a filtered water bottle station in the food preparation area. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPAs observed all hazardous items to be inaccessible to children. Isolation area will be in the director's office. There is a separate restroom for children's use in the director's office that is labeled for children's use. Facility will provide morning and afternoon snacks only. The kitchen area currently includes a refrigerator, and a sink. The kitchen area was observed to be free of rodents. Food was observed to be properly stored, separate from cleaning materials. The staff shall ensure that each child has an individual permanent or portable storage space for his/her clothing, personal belongings and/or bedding. All materials and surfaces accessible to children, including toys, shall be free of toxic substances. This facility plans to provide Incidental Medical Services \342\200\223 IMS. There is a separate outdoor/patio area for teacher's use only and is made inaccessible to children. Shade is provided via a permanent awning. There is a climbing structure on the playground that is properly anchored. There is adequate cushioning in fall zones of the climber by soft padding. LPAs did not observe any pools, spas or bodies of water on site. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Any changes to facility, the department must be made aware prior to the changes and must be approve by department. Per Title 22, Section 101212(c) The licensee shall notify the Department in writing of his/her intent prior to making any structural changes that reduce the total amount of indoor or outdoor activity space. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPA reviewed with applicant and head teacher, the LIC 311A, Records to Be Maintained at The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication Prior to licensure, following is to be completed: -Installation of child safety gate to teacher/food preparation area. -Locking of teachers' cabinets. -Garbage bins with tight fitting lids throughout facility. -Installation of diaper cabinet in Blueberry classroom. -Installation of padding on pole in Blueberry classroom. -Sound alarms on all exit doors in each classroom. -Posting of licensing documents in each classroom (due to separate sign in/out areas). -Installation of separate carbon monoxide detector in facility. -Removing of stall doors on children's bathroom stalls. -Cleaning of outdoor area. -Final set up of each classroom following Title 22 regulations. Applicant, Grace Yee, understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
