What the inspector wrote
On 1/21/2026, Licensing Program Analyst (LPA) Harsimran Kaur met with Natella Shtern, Applicant representative. The purpose of the visit was an announced change of location pre-licensing inspection. This facility has an existing preschool program #414004671 and infant program # 414004888. Facility requested to be licensed for total of 33 children consisting of 8 infants (ages 6 months to 24 months) and 25 preschool children ( ages 2 to 5 years). The child care center will operate Monday\342\200\223Friday 7:30 AM \342\200\223 5:30 PM. The fire clearance was done on 9/30/2025. The facility is located at a Open Mind School grade K - 12. School hours are 9 AM to 3 PM.
LPA toured and measured the on-limits areas and classrooms located at the facility. The facility has a total of 2 classrooms.
Preschool Classrooms measurements are:
The Indoor total measurements contain a total of {891.19} square feet, which will accommodate Applicant's request for 25 preschool children.

Outdoor total measurements contain a total of {854.25} square feet, which will not accommodate Applicant\342\200\231s request for 25 preschool children.
Facility does not have separate yard for infants. Applicant will submit the waiver.
LPA observed all classrooms to be clean, safe, sanitary and in good repair. A comfortable temperature for children shall be maintained at all times. Furniture and equipment were observed to be maintained in good condition, free of sharp, loose or pointed parts. All materials and surfaces accessible to children, including toys, shall be free of toxic substances. LPA observed fully charged fire extinguisher, smoke and carbon monoxide detectors in the facility. There are 20 chairs, 4 tables, 25 cubbies, 2 toilets, 2 sinks 6 feeding chairs, 3 cribs and 2 changing table available for infant and preschool children. Staff have separate bathroom that can be utilized by sick children if needed. Sick children will be isolated in the office area of the facility. LPA observed that items that may pose a danger to children were stored in high cabinets inaccessible to children. LPA observed containers with tight-fitting lids for the disposal of solid wastes. LPA advised that Applicant maintains first-aid supplies in a location accessible to staff but inaccessible to children.
The Applicant shall take measures to keep the center free of flies, other insects, and rodents. Drinking water is readily available for the children via water gallons and children can bring their own water bottles outdoors. Applicant stated that facility provides snacks to the children. Lunch is provided by parents.
To minimize disruption, preschool children may use the designated pathway from the preschool room through the infant room to access is strictly limited to restroom use only. Preschool children should not interact with infants or use any infant equipment while in the infant area. Additionally preschool children must be accompanied by preschool staff at all times when entering or passing through the infant area. The facility understands and agrees to follow this plan. Any proposed changed must be submitted to the Regional Office for review and approval prior to implementation.
Facility has one infant classroom. Applicant stated that parents will provide milk bottles for infants.. LPA advised Applicant that as per Title 22 regulations, bottles and dishes shall be rinsed for sanitizing using facility dishwasher at the end of each day.
Outdoor area for preschool children were free of hazards including, holes, broken items, dry grasses, and other debris that pose a fire hazard. Shade was provided by building overhead. Children will pass through the school gym to access the yard. The playground is enclosed by a fence to protect children and to keep them in the outdoor activity area. LPA observed no bodies of water. The fence is at least four feet high. Any construction or equipment that could pose a hazard in the outdoor activity area shall be made inaccessible to children in care. Per applicant there are no weapons within the facility premises.
Drinking water from a non-contaminating fixture or container shall be readily available both indoors and in the outdoor activity area. LPA advised per Health and Safety Code 1597.16(a)(1) Lead Testing and PIN-21-21-CCP Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every 5 years after the date of the first test. Per Written Directive 100700(c)(1) Written Directives for Lead Testing (1) For a license issued on or after July 1, 2022, initial testing results shall be received and posted within 180 days of licensure.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at:

, as an additional resource. LPA also informed Applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at:

, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
LPA reviewed with the Applicant LIC 311A, Records to Be Maintained At The Facility, child\342\200\231s records, personnel records, administrative records, and documents to be posted.
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies(R&Rs) throughout California.

The following corrections are needed before licensure will be considered.
1. Approval of outdoor waiver to share preschool yard with infants.
An exit interview was conducted and copy of appeal rights were provided to Natella Shtern, Applicant Representative. Final license determination for a single license will be made upon review by the Licensing Program Manager.
