On Monday May 12, 2025 at 10:00 AM Licensing Program Analyst (LPA) Elizabeth George conducted an unannounced case management incident inspection at the above-mentioned facility. LPA met with Program Director, Michele Tappeiner, and discussed the purpose of the inspection was to follow up on an incident reported to the Department on May 6, 2025 as required. LPA was escorted through the facility and toured the inside and outside. During the inspection, LPA observed 5 staff providing care to 19 children. LPA received an Unusual Incident Report about a narrative from a child in care told to the parents of said child. Parent took their concerns to the program director whom reported to the department. LPA toured the area where the incident was said to have occurred and interviewed program director and two teachers. Program director described where teachers are stationed during outdoor play. LPA toured the bathrooms as well and discussed the bathroom protocol. Based on the information gathered during the inspection, LPA determined that the program director took appropriate action to meet the needs of child 1 and other children in care. Given the incident was appropriately handled, no deficiencies are being issued as a result of this incident. Notice of site visit was issued and should remain posted for 30 days. Exit interview was conducted and report was reviewed with Program Director, Michele Tappeiner.
