On 12/19/2025 Licensing Program Analyst (LPA) German Negrete made an unannounced visit for the purpose of completing a Case Management - Incident inspection. Today LPA met with Center Site Supervisor Susana Del Toro. LPA conducted a walk through of the Child Care Center(CCC) with the Center Site Supervisor. LPA observed at the time of the walk-through 4 toddlers being supervised by 1 staff. The purpose of the case management inspection is due to multiple incidents involving C1. On 12/10/2025 C1 was dropped off at the center by parent(P1). Then (P1) opened up a faucet to a water fountain that was meant to be shut off at the CCC. The parent allowed C1 to consume the water that came out of the faucet. According to S2, the water came out brown. Another incident occurred on 9/16/2025, when a parent(P3) contacted, the police due to the rough manner P2 allegedly handled C1. P3 contacted law enforcement, police arrived at CCC and spoke with the both families(P2 and P3). CCC representative filed/wrote a report for CWS. Today LPA interviewed staff. LPA also verified the LIC624s for both aforementioned incidents were submitted in the appropriate time frame as outlined in Title 22 California Code of Regulations. Continued On LIC809-C Today LPA requested a children roster. The review of the two incidents is on going. Exit interview conducted with the Site Supervisor Susana Del Toro. Appeal rights were provided. THE NOTICE OF SITE VISIT MUST REMAIN POSTED FOR 30 DAYS OR A CIVIL PENALTY OF $100.00 WILL APPLY.
