What the inspector wrote

On 07/03/2025, Licensing Program Analyst (LPA) Bill Billones conducted an unannounced Case Management inspection of the above-mentioned Child Care Center (CCC) to follow up on an Unusual Incident Report (UIR) received by the Department on 05/22/2025. Specifically, a child in care (C1) sustained an injury to the arm and head that required medical treatment. The injuries occurred in the outdoor play area where C1 fell down from the play structure. LPA met with CCC Site Supervisor Susana Del Toro and explained the nature of the inspection. LPA notes 12 children are on site along with 3 teachers providing care and supervision. LPA and Site Supervisor viewed the area where C1 injured their arm and bumped their head. LPA notes the area is within the CCC's outdoor playground. LPA observed the play structure has cushioned material (rubber) to absorb falls. Site Supervisor informed LPA the incident occurred when C1 slipped from the play structure and fell down. Staff (S1) and another staff member had witnessed the incident. C1 was attended by staff, authorized representatives were informed, and child was taken to their pediatrician. Site Supervisor informed LPA that C1 has since returned to the center. Site Supervisor also stated the center adheres to the U.S. Department of Health and Human Services Standards for Playground Use Zone Safety. Based on the information gathered during the inspection, LPA determined that the center’s staff took appropriate action to meet the needs of C1. No deficiencies were cited as a result of the incident. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Site Supervisor, Susana Del Toro.
