What the inspector wrote

On January 29, 2026 at 9:29 AM, Licensing Program Analysts (LPAs) Bill Billones and Izak Diaz conducted an unannounced Case Management inspection of the above-mentioned Child Care Center (CCC) to follow up on an Unusual Incident Report (UIR) received by the Department on 01/16/2026. LPAs met with Site Supervisor Susana Del Toro and explained the purpose of the inspection. LPAs noted 15 children present under the care and supervision of 4 staff. LPAs and the Site Supervisor toured the center both inside and outside. LPAs reviewed two children’s files for child #1 (C1) and child #2 (C2). LPAs interviewed the Site Supervisor regarding the incident and Site Supervisor provided a copy of the medical report for C1. LPAs note the Site Supervisor reported the incident in accordance with Title 22 regulations for reporting requirements. Based on the information gathered during the inspection, LPAs determined that the Site Supervisor took appropriate action to meet the needs of both C1 and C2. During today’s inspection, no deficiencies are being cited as a result of the incident. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Site Supervisor Susana Del Toro.
