What the inspector wrote

On April 1, 2025 at 2:20 PM Licensing Program Analyst (LPA) Elizabeth George conducted an unannounced case management inspection to follow up on incidents reported on 8/29/24, 9/25/24, 10/24/24 and 2/5/25. LPA met with Lead Teacher (LT) and explained the purpose of the inspection. Together with the lead teacher LPA toured the facility in its entirety. Incident reported 8/29/24 - C1 bit C2 on the upper arm. LT explained the process they have for bites with children. Stating they check the child's bite. Make sure the child is ok. They offer ice to the child. They report to the parents and the parent signs an ouch report at pick up. C2 skin was not broken from the bit on this day. Incident reported on 9/25/25 - C1 bit C2. Process for biting was completed. Child was fine with no broken skin. Incident reported on 10/24/24 - Child went to sit on a wooden bench, missed and hit lip on bench, splitting lip. Child's lip was cleaned up and child was offered ice. No medical attention was needed beyond that. Parents were informed and ouch report was signed. Incident reported on 2/5/25 - City shut off water. CCC was notified of the water being shut off but no letter was sent to them. Upon teachers arrival at facility there was no water. CCC staff notified the parents of the centers closure for the day and gave the parents the option of taking their chidlren to the "I" Street center. Parents transported their own children to the other center. It has been determined that appropraite action was taken for the above incidents and no defieciences are being cited. Exit interview was conducted and report was reveiwed with Lead Teacher, Yasmin Diaz.
