What the inspector wrote

On December 29th at 9:45am, Licensing Program Analyst (LPA) Brian Fung made an unannounced inspection for the purpose of conducting a Case Management inspection. LPA met with Cynthial Horton to discuss construction for an HVAC system and temporarily relocation is needed that was self-reported to Community Care Licensing Division (CCLD) office by phone on 12/19/2025. LPA toured the area/classroom where the construction occurred and interviewed the site supervisor. At the time of the inspection there were 3 teachers and 1 site supervisor with 7 children. On 12/19/2025, LPA received an email discussing the facility stated above will be temporarily relocating to "I" Center for one day on 12/22/2025. During interview with program director and site supervisor, it was found that both administrators called SBRO licensing office on 12/19/2025 at 1:04pm and left a voicemail. On 12/19/2025 at 1:07 pm, Program Director and site supervisor spoke with Licensing Program Manager (LPM) regarding about the temporary closure of 12/22/2025 and relocation to "I' center "located 4 blocks down the street." LPM granted permission to relocate for one day as long as the site supervisor carries the current children files, teacher files, and all licensing facility required documents to the relocated facility. Given the relocation was granted by LPM and site supervsior and program director took appropriate action by calling SBRO licensing office and LPM, no deficiencies are being issued as a result of this incident. Exit interview and review of report was conducted with site supervisor Cynthial Horton. Notice of site visit were provided and must remain posted for the next 30 days.
