What the inspector wrote

On May 20, 2026 at 1:52 PM, Licensing Program Analysts (LPAs) Bill Billones and Izak Diaz conducted an unannounced Case Management inspection of the above-mentioned Child Care Center (CCC) to follow up on an Unusual Incident Report (UIR) received by the Department on 05/19/2026. LPAs met with Director Melanie Newhouse and explained the nature of the inspection. LPAs and the Director toured the center and observed the area were the incident occurred. LPAs noted 111 children present under the care and supervision of 18 staff, including the Director. LPAs interviewed the Director regarding the incident. Child #1 (C1) was injured on a play structure, which required medical attention. Based on the information gathered during the inspection, LPAs determined that the Director took appropriate action to meet the needs of children in care. The UIR occurred on 05/13/2026 and the Director did not report the incident in accordance with Title 22 regulations for reporting requirements. LPAs reminded Director to report incidents requiring medical attention to the Department within 24 hours. During today’s inspection, no deficiencies are being cited as a result of the incident. 1 technical advisory has been issued. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Melanie Newhouse.
