What the inspector wrote

On October 1, 2025 Licensing Program Analyst (LPA) Elizabeth George conducted an unannounced Case Management Incident Inspection. LPA met with Director, Artesia Carlon, and discussed the purpose of the inspection, which was to follow up on two incidents reported to the Department on 5/29/25 & 9/22/25. LPA along with Director toured the facility inside and outside. During the inspection there were 14 children in care of 3 staff. Summary of UIR dated 5/29/25 stated: Child was walking in the library area during clean up, tripped, fell and hit lip on the wooden couch. Staff applied first aid and called mother. Furniture in the library is age appropriate and in proper conditions. Child was picked up and taken to the doctor where he received medical treatment. Child was cleared by doctor to return the following day. Summary of UIR dated 9/22/25 stated: C1 reported C2 to have touched them inappropriately multiple times in the bathroom and library area. Children are escorted to the bathroom where only one child is allowed in the restroom at a time. Staff log each child's name and time when the restroom is used. Ratio is 1:8 in the classroom on a daily basis. Library is open to the rest of the classroom and can be seen from all angles. Director has increased the teacher child ratio and is having C2 shadowed at this time. LPA spoke with staff about children in report. SBCEO is conducting an internal investigation of the incident as well. Based on the information gathered during the inspection, LPA determined staff took appropriate action to meet the needs of children in care for both incidents. No deficiencies were cited as a result of the incidents. A Notice of Site Visit was given, exit interview conducted and report was reviewed with Director, Artesia Carlon.
