On May 19th, 2026 at 9:30Am, Licensing Program Analyst (LPA) Brian Fung made an unannounced inspection for the purpose of conducting a Case Management inspection. LPA met with Arielle Powell to discuss an incident that was self-reported to Community Care Licensing Division (CCLD) office by phone on 5/18/26. LPA toured the classroom where the incident occurred and interviewed site supervisor. GIven the incident was only observed by 1 staff member, LPA interviewed 1 staff regarding the incident. The incident is in regards to a child who was injured from falling down onto a chair and required emergency medical treatement. Upon further investigation, the child was jumping on the floor and was holding the chair. The child fell forward and hit their chin onto the chair. Following the incident C1 continues to be enrolled in the CCC. Staff took appropriate action by providing active supervision and immediately tended to the child with gauze and tape. The staff provided comfort to the childt while contacting parents for updates. Given the incident was observed by staff, no deficiencies are being issued as a result of this incident. Exit interview and review of report was conducted with Site Supervisor Arielle Powell. Notice of site visit and appeal rights were provided and must remain posted for the next 30 days.
