What the inspector wrote

Interviews revealed parent's are always informed of injuries right away by means of phone call or text message. Parents also reported being provided with incident reports with details of the incident. Parents expressed there is a lot of communication between staff and parents. Therefore, parent interviews revealed families are satisfied with the CCC and did not express any concerns regarding the CCC. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations is UNSUBSTANTIATED. A Notice of Site Visit (LIC 9213) and Appeal Rights (LIC 9058) were provided to the Director. The Notice of Site Visit must remain posted for 30 days or a civil penalty of $100.00 may appeal.

This requirement was not met as evidence by: LPA's review of Guardian and LIS identified S2 is associated to the CCC but is currently reflecting "In Process" Per Director, S2 has been employed in the facility since about September 2023 and Director, was under the impression that S2 was cleared and associated to the CCC.
