What the inspector wrote

On February 10, 2022 at 12:45 PM, Licensing Program Analyst (LPA), Martina Jimenez, conducted an unannounced Case Management \342\200\223 COVID-19 inspection and met with, Cecilia Leon, Licensee. LPA informed the purpose for the inspection and completed a COVID-19 pre-screening questions prior to the commencement of the inspection. Licensee gave LPA a of the facility, inside and outside. This inspection is a follow-up on COVID-19 outbreaks reported by facility licensee to CCL between 1/16/2022 - 1/17/2022. CCL provided a Tele - Rapid Assistance Support Team (RAST) inspection on 10/6/2020, where LPA provided COVID-19 resources, current Childcare Industry Guidance, and a COVID-19 self-assessment guide to the facility. LPA observed facility staff wearing masks upon arrival to the facility. Children in care also had masks on during this inspection. LPA discussed facility\342\200\231s written plan for when a staff member or child tests positive for COVID-19 and developing a written communication plan with parents/guardians to share information and guidelines in their preferred language. LPA discussed COVID-19 Face coverings, Essential Protective Equipment and Supplies, Physical Distancing, Cleaning and Disinfection, Hand washing, Food Service and Mealtimes, Directed to contact local Public Health for Positive cases and concerns. LPA reviewed that facility maintains a current Covid-19 Child Care Program Self-Assessment Guide/Mitigation plan. LPA also provided the following resources: This Report Continues on LIC 809C - Official Public Health and Child Care Guidance for COVID-19 \342\200\223 Updated 1/26/2022. - COVID-19 Child Care Resources: https://www.cdss.ca.gov/inforesources/cdss-programs/community-care-licensing/child-care-licensing/covid-19-child-care-resources There were no deficiencies cited during today's inspection. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee, Cecilia Leon.

    Based on review of infant records, an individual infant sleep plan was never completed & signed by the child's authorized representative. For infant #3, which poses a potential Health, Safety or Personal Rights risk to persons in care.

    Based on LPA review of child's records and interview with the licensee revealed that licensee discontinued on documenting on 10/29/2021, the every 15 minute supervision of sleeping infants - Child 3 . This poses a potential risk to health and safety of children in care.

    Based on LPAs observation, interview with licensee and record review, Licensee stated that Ricardo Fragoso, licensee's husband has been immunized against pertussis and measles, but did not have a record of immunization or other proof of immunity. Licensee stated that licensee's husband declines the infuenza vaccinne. Licensee did not ensure to have a copy licensee's husband immunization record on file which poses a potential risk to the health and safety of children in care.the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.
