What the inspector wrote

On April 1, 2026 Licensing Program Analyst (LPA) Elizabeth George conducted an unannounced Case Management Inspection. LPA met with Site Supervisor, Veronica Rubio, and discussed the purpose of the inspection was to follow up on an incident reported to the Department on 3/6/26 as required. LPA in the company of the site supervisor toured the facility inside and outside. During the inspection, LPA observed 3 staff providing care to 16 children. On March 5, 2026, during breakfast time, a child got up to throw away trash from meal, when the child turned around, the child ran back to the table and slipped and fell. The child's left cheek hit the side of the bench where the children sit. The child sustained a mark in the cheek and was provided an ice pack. The child returned to school the next day with a bruised cheek and a black eye. Child was seen by the doctor and cleared to return to school. Site supervisor made changes within the lunch area by moving trash cans closer to the tables and talked with the children about walking feet and being safe. Based on the information gathered during the inspection, LPA determined that the staff took appropriate action to meet the needs of the child and other children in care. No deficiencies were cited as a result of the incident. Exit interview conducted and report was reviewed with the Site Supervisor. A notice of site visit was given.
