What the inspector wrote

On 6/23/22, at 9:15 AM, Licensing Program Analyst (LPA) Elvin Baddley conducted an unannounced One Year Required inspection of the abovementioned Child Care Center (CCC). LPA met with Shelely Grand, Director of the CCC, and explained the purpose of the inspection. LPA, in the company of the Director toured the interior and exterior of the CCC. The CCC operates out of two classroom and an outdoor play area at the noted address. The CCC's hours of operation are from 7:30 AM to 5:00 PM, Monday - Friday. During the inspection, LPA observed 34 children in care, along with four teachers (clear and associated) providing supervision. All staff members were wearing face coverings at the time of the inspection The classrooms used for child care are clean and organized. The CCC has storage space ("cubbies") for all children in care near the entry of the facility and age appropriate furnishings, toys and equipment throughout. The CCC has an operable refrigerator in the kitchen area which is inaccessible to children in care. The refrigerator contains food items consumed by the CCC's children which are labeled and dated. The CCC provides snacks and lunches to children in care. LPA notes available bottled drinking water for children inside and outside. CCC does not use water fountains on site. Further, fountains are label out of use and covered. LPA informed Director of requirement to have CCC's water sources lead tested. LPA observed menus posted at the CCC along with required notices and documents near the facility entry door. The facility uses the Bright Wheel Application to sign children in and out of the CCC. LPA reviewed CCC Sign In/Out information and notes all information reviewed were found to be completed appropriately, having the authorized parent's electronic signature as well as entry and exit times. The CCC also has paper sign in/out forms available in the event the Bright Wheels Application is not operable LPA observed a carbon monoxide detector on site which was tested at 10:50 AM and found to be operable. Disinfectants and cleaning solutions are stored in cabinet in the CCC's laundry room which is inaccessible to children in care. Sharps and medications at the CCC are out of the reach of children in care and inaccessible. (CONT 809-C) The restrooms of the CCC, two in total, are observed to be clean and void of toxins. Waste receptacles at the CCC are observed to have tight fitting lids. The outdoor playground area is enclosed by cider block and wooden fencing. The fence\342\200\231s entry/exit gates are secured and furnishings, toys and equipment in the outdoor area is age appropriate. LPA observed locked sheds in the outdoor play area which contain CCC supplies and equipment. The play structures in the outdoor play area have adequate cushioning materials to absorb falls. Shade is provided by trees, shade covers and building overhangs. LPA observed water tables in the outdoor play area. Director informed LPA the water tables are empty daily and cleaned. Children using water tables are directly supervised. Play structure are absent of sharp, pointed or loose parts. Director was reminded to replace play equipment once the play equipment begins to degraded. Staff and children records were reviewed. The children\342\200\231s records were complete and found to contain emergency contact information as well as immunization records. Staff records were reviewed and contained current pediatric CPR and First Aid certifications (EMSA) along with Mandated Reporter certifications.Director reminded to keep training certifications current and updated prior to expirations. Director notes no firearm or ammunition is stored on site. LPA notes the last fire/disaster drill occurred on 5/3/22 . Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility representative Shelley Grand.
