What the inspector wrote

On March 27, 2026, Licensing Program Analyst (LPA) Fernando Hernandez conducted an unannounced Case Management - Incident Inspection. LPA met with Licensee Maribel Ambrose and (1) assistant and discussed the purpose of the inspection was to follow up on an incident reported to the Department on 03/12/26 as required. LPA in the company of the licensee toured the facility. During the inspection, LPA observed licensee and assistant providing care to (5) children. On 03/12/26 at approximately 1:27 PM, children were sleeping when A2 noticed C1 was 'shaking'. A2 stated they tried to soothe the child believing they were having a nightmare, A2 decided to speak to C1 and noticed C1 was not responding. A2 stated they proceeded to call 911, while staying close to the child. A2 acted immediately making sure not to move the child. 911 and parents were called immediately. Approximately 3 minutes after the paramedics arrived and had take the C1 was taken to the emergency room for observation. Licensee stated they went with C1 to the emergency room until parent arrived. Licensee stated they Child was discharged from hospital and stayed with parent the same day. Licensee stated C1 returned 03/14/2026. Licensee stated the parents informed licensee the test performed by the doctors came out good. Based on the information gathered during the inspection, LPA determined that the licensee and assistant took appropriate action to meet the needs of the child and other children in care. No deficiencies were cited as a result of the incident. Exit interview conducted and report was reviewed with the Licensee Maribel Ambrose. A notice of site visit was given.
