On February 13, 2026 at 9:45 AM, Licensing Program Analyst (LPA) Brian Fung conducted an unannounced annual inspection. LPA met with licensee Luis Carlos Ayala and advised the purpose of the inspection. Licensee provided LPA a tour of the home inside and out. There were 2 children in care at the time of the inspection and 3 adults in the home. 2 adults were staffed to care for children (licensee and assistant/wife) LPA observed required Licensing required documents posted prominently at the entrance. Fire and earthquake drills are being documented every six months. Last drill was conducted on 12/16/2025. Fire extinguisher 2A10BC was found in the dining room shelf and was last purchased in August of 2024. Licensee was reminded that fire extinguisher requires to be replaced or serviced annually. A type B deficiency was cited for not replacing the fire extinguisher yearly. Fire and carbon monoxide detectors were tested at 10:17 AM and found to be in working condition. Children in care have access to the living room, TV room, 1 bathroom, 2 bedroom for sleep, and 1 backyard. Fireplace inside the living room is screened to prevent access by children. Children do not have access to the kitchen. Children have access to toys that are age-appropriate inside and outside of the home. Back yard play area is enclosed and has plenty of play structures, toys, and activities. There are no guns and/or ammunition in the facility. There are no bodies of water present at the time of inspection. During the tour, room spray, perfume, and skin care items were located in the children's bathroom that was made accessible to children. A type B deficiency was cited for making chemical agents accessible to children. Continued on 809-C A roster of children in care was observed current and complete. LPA reviewed five out of five children files. All children files were current. Licensee CPR/first aid is valid through 1/21/2027. Licensee was reminded a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter certification AB1207 every two years at www.mandatedreporterca.com Licensee\342\200\231s mandated reporter certification is valid through 9/7/2026. Upon review of assistant/wife's file, a TB test and immunization record is missing. A type B deficiency was cited on today's visit for missing required immunization record and TB test made available during the inspection. Licensee updated LPA that at 3:15 PM today, assistant/wife made an appointment to obtain the immunization records and conduct the TB test. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Continued on 809-C Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the LICENSEE Luis Carlos Ayala, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. There were 3 Type B deficiencies cited during today\342\200\231s inspection. See 809-D for more information. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Appeal rights were given.
