What Happened
The unannounced annual inspection found multiple safety and records issues, including an expired fire extinguisher, accessible toxins, and incomplete staff and child files. A new staff member also lacked a file and was not fingerprint cleared or associated to the facility.
Observations
- Chimney screen in the play area was not locked and was accessible to children
- Toxins and cleaning supplies were stored in an accessible bathroom cabinet
- Fire extinguisher was expired
- Knife and adult scissors were left accessible in the kitchen and dining area
- Staff member S2 had no file and was not fingerprint cleared or associated
- Children's files for C3, C6, and C9 were missing immunization records or blue cards
- Fire and disaster drill was not conducted and logged within the last 6 months
