What the inspector wrote

On 08/24/2023 at 10:45 AM, Licensing Program Analyst (LPA) Francisca Velazquez conducted an unannounced Required Annual Inspection of the Family Child Care Home (FCCH). LPA met with Laura Alapizco, licensee of the FCCH. LPA discussed the purpose of the inspection and toured the interior and exterior of the home. LPA observed 14 children present during today's inspection. At 10:45 AM, LPA observed 14 children present and notes only one child is school age. At 10:45 AM, LPA observed licensee, S2 and S3 offering care and supervision to children in care. LPA observed S5 in the home but not providing care and supervision. At about 11:15 AM, LPA observed S4 arrive to the home. Licensee reported S4, is her adult child and lives in the home. Per guardian, S4 is not fingerprint cleared as S4 reflects her criminal record clearance is in process. This is a single-story home that consists of three (3) bedrooms, two (2) bathrooms, living/dining room, kitchen, laundry closet, garage, and outdoor yard. The main day care areas are living/dining area, kitchen, bathroom, and outdoor yard. Meanwhile, the garage, three (3) bedrooms, one (1) bathroom, laundry closet and part of the outdoor yard are inaccessible to children in care. LPA observed the day care area to be clean. LPA observed there is enough spacing for the comfort of children present today. LPA observed there is ventilation to afford the children in care. LPA observed age-appropriate books, toy, games, tables, and chairs for children in care. In the kitchen, LPA observed that sharps are stored in an elevated cabinet and in a locked drawer. LPA observed cleaning compounds are stored in an elevated cabinet in the kitchen, the garage and in the laundry closet. LPA observed medication is stored in an elevated cabinet in the kitchen and licensee\342\200\231s bedrooms. LPA notes the garage is made inaccessible by means of a door safety knob and bedrooms are locked and secured with a gate. CONT 809-C LPA and licensee toured the exterior of the FCCH. LPA notes the outdoor yard is completely fenced. In addition, LPA observed the sides of the outdoor yard are inaccessible to children by means of fences. LPA observed there is plenty of shade provide by mesh that has been added to the outdoor yard. LPA observed plenty of age-appropriate toys, chairs, and tables for children. Filtered water is accessible to children during the day by means of individual water cups. LPA and licensee discussed active supervision when children are outdoors. Licensee stated that there are no weapons/ammunition in the home. Licensee stated she does not hold a foster family license. LPA observed licensee test smoke alarm at 11:30 AM and was operable. At 11:30 AM, licensee reported she does not have an operable carbon monoxide detector. LPA observed a fire extinguisher that was serviced on 06/12/23. Licensee was reminded to service or purchase a fire extinguisher annually. LPA reviewed emergency drill log and found the last emergency drill was conducted on 3/02/23 which is within 6 months of today\342\200\231s date. At around 11:45 AM, LPA conduct record review of licensee and personnel. LPA\342\200\231s record review revealed, licensee does not have personnel file for licensee, S2, S3 and S4. In addition, licensee, S2, S3 and S4 do not have required immunizations accessible for review. LPA\342\200\231s file review revealed S2, S3, and S4 have current mandated reporter training certificate but licensee does not have current mandated reporter training certificate readily accessible for review. At around 12:30 PM, LPA conducted children record review and found children files to be incomplete. LPA\342\200\231s record review revealed C6-C14 did not have any files at all in the FCCH for review. In addition, C1, C2, C3, C4, and C5 had incomplete children files in the FCCH. LPA found C1 did not have emergency card, emergency consent form or immunizations. LPA found C2 had a file with documentation, but the documentation was not filled out or signed by guardian. LPA found C3 had a file with documentation, but the documentation was not filled out or signed by guardian. LPA found C4 had a file with emergency card incomplete and not signed by guardian. LPA found C5 had a file with documentation, but the documentation was not filled out or signed by guardian. In addition, record review revealed C6 and C14 are under the age of one year and did not have Individualized Infant Sleep Plan, LIC 9227 in their file for review. Record review revealed licensee is not documenting 15-minute checks for C8, C6 and C14, all infants are under the age of two years. Lastly, LPA\342\200\231s record review revealed licensee has facility roster but out of the 14 children present today only two children were documented in the facility roster. CONT 809-C Licensee is not providing Incidental Medical Services. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA provided a Handout for Reporting Child Abuse and Neglect Training provided on line at www.ccld.ca.gov . Licensee was reminded that it is her responsibility to know the regulations for Family Child Care Home and was advised to review Quarterly Updates and Provider Information Notices (PINs), Title 22 & Health & Safety Codes which can be accessed on-line at https://www.cdss.ca.gov/inforesources/child-care-licensing Today, deficiency cited under Title 22 Division 12, civil penalty access and Appeal rights given. Upon receipt of this report, licensee shall post and provide copies of this licensing report to parents /guardian of children in care at the facility and to parent/guardians of children newly enrolled at the facility during the next 12 months. Licensee to provide LIC 9224 for each child in care and have each parent sign the form that they have received a copy of the report. Exit interview and review of report was conducted with licensee, Laura Alapizco. Notice of site visit and appeal rights were provided to licensee.

    Based on observation and record review, the licensee did not comply with the section cited above in that S1, S2, S3 and S4 do not have personnel files in the FCCH to review which poses/posed a potential health, safety or personal rights risk to persons in care.

  • STAFFING RATIO AND CAPACITY

    Based on observation and record review, the licensee did not comply with the section cited above in that there were 14 children present with only one being a school-age child which poses an immediate health, safety or personal rights risk to persons in care.

  • OPERATION OF A FAMILY CHILD CARE HOME

    Based on observation and record review, the licensee did not comply with the section cited above in that C6-C14 did not have any emergency card in the FCCH. Additionally, C1, C2, C4, C5 had an emergency card that was not completed or signed by the guardian which poses/posed a potential health, safety or personal rights risk to persons in care.

  • OPERATION OF A FAMILY CHILD CARE HOME

    Based on observation and record review, the licensee did not comply with the section cited above in that 12 out of the 14 children present during inspection were not documented on the facility roster which poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on observation and record review, the licensee did not comply with the section cited above in that C6-C14 did not have any immunizations on file, C1, C3 and C5 also did not have current immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.
