What the inspector wrote

This is an amendment report to reflect the toddler option has been granted. On December 28, 2022 at 8:45 am, Licensing Program Analyst (LPA) Susana Martinez conducted an unannounced Case Management- Other inspection to deliver a waiver and grant the toddler option. LPA met with site director Lucia Torres and explained the reason for the inspection. Director and LPA toured the facility inside and outside. At the time of the inspection there were 3 infant children in care along with 2 staff members. During the tour, the LPA did not observe any hazardous/toxic items. Required documents were observed to be posted. LPA also reviewed the check-in/check-out log. The center submitted a waiver request to allow the toddler program to use the infant playground on a staggered schedule. The waiver request has been granted and is valid for the term of the license and is subject to review at the discretion of the licensing agency. The waiver was granted under the following conditions: 1. The center shall abide by the following schedule for outdoor activity space at all times unless a revised schedule is approved by the Department. Infants Schedule: Mon-Fri :9AM- 10:30 AM and 3:05 PM- 4 PM Toddlers Schedule: Mon- Fri :7AM- 8:30 AM, 10:30 AM- 11:30 AM and 4 PM- 5 PM 2. Infants and toddler programs cannot be commingled at any time during the hours of operation. 3. The center must ensure appropriate equipment use and supervision of children by a qualified teacher at all times. 4. The center shall be able to meet the needs of all children in care at all times. Failure to comply with the conditions may result in the termination of the waiver. The waiver must be posted with your license. The center was granted the outdoor staggered schedule waiver and the toddler component. Infant capacity is 4. Toddler capacity is not to exceed 8 children. Exit interview conducted, appeal rights were given, and report was reviewed with the director, Lucia Torres. Notice of Site Visit was given, failure to post may result in $100 fine.
