Follow-up on corrections·June 4, 2026

On June 4th, 2026 at 12:31 PM, Licensing Program Analysts (LPAs) Fernando Hernandez and Nicole Ketterer conducted an unannouced Case Management Deficiencies Inspection at the above-mentioned Family Child Care Home (FCCH). Upon arrival, LPAs met Licensee Cynthia Arreola and informed them of the purpose of the inspection. LPAs observed 4 children present with Licensee providing care and supervision. LPAs toured the home inside and outside. Upon inspection, LPAs observed the outdoor play area with animal droppings and broken play equipment such as plastic seat on tricycle was observed with broken sharp edges. During the inspection, LPAs observed debris/dirt on outdoor play equipment. Additionally, LPAs observed children's couch with a hole exposing the frame of the couch in the accessible outdoor play area. Lastly, LPAs note licensee stated the children were outside earlier in the morning today. LPAs reviewed 4 out of 4 children's records and observed (1) child's file was incomplete. LPAs note children's LIC627 form was missing from child's file. In areas evaluated, (1) Type B deficiency is being cited under Title 22 California Code of Regulations 102417(g)(4), which can be found on the attached LIC809-D. Additionally, (1) technical violation was cited based on record review and observations. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted, appeal rights provided, and report was reviewed with Licensee Cynthia Arreola.
