What the inspector wrote

On December 12th at 8:45 am, Licensing Program Analyst (LPA) Brian Fung made an unannounced inspection for the purpose of conducting a Case Management inspection. LPA met with Site Supervisor Meghan Stallworth to discuss an incident that was self-reported to Community Care Licensing Division (CCLD) office by phone on 12/11/2025. LPA toured the area/classroom where the incident occurred and interviewed site supervisor and 2 teaching staff. At 9:30 am, LPA conducted interviews with site supervisor and S1 and S2 to obtain information of the incident. Based on interviews, it was found that the child was left unattended in the classroom for 19 minutes resulting to an absent of supervision during rest time. During the interview, it was found that stacks of chairs were blocking the visual observance of the child sleeping while transition the children to outdoor play time. During today’s inspection, an immediate risk was found for absent of supervision. A type A violation was cited for today's inspection with an immediate civil penalty. LPA Brian Fung informed site supervisor Meghan Stallworth that this report dated 12/12/2025 document(s) 1 Type A citation. Type A citation(s) which shall be posted for 30 consecutive days as an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Brian Fung informed the site supervisor to provide a copy of this licensing report dated 12/12/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview and review of report was conducted with site supervisor Meghan Stallworth. Notice of site visit and appeal rights were provided and must remain posted for the next 30 days. Appeal rights were given. Type A Violation must be remain posted for 1 year and current and potential families enrolled must signed acknowledgment agreement.

    (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, Supervision shall include visual observation.This requirement is not met as evidence by: Based on interviews and record review, the teachers left a child unattended inside the classroom for 19 minutes during rest time resulting to absence of supervision. The child was crying and found by a teacher assistant which poses an immediate Health and Safety andPersonal Rights risk to persons in care.
