What the inspector wrote

Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Required 1- Year inspection. LPA met with Site Director Perlina Chavez and explained the reason for the inspection. Present during today's inspection were 6 children and at least 3 staff. There is a board to post required postings. The hours of operation are Monday through Friday 7:30AM to 5:30PM. Facility has a waiver to commingling the infant and preschool program in Room 4 from 7AM to 8AM and 4PM to 5PM. Site Director stated that she understands the conditions of the waiver and cannot commingle the two program beyond that time frame. Site Director stated that she will follow-up with Licensing in regards to submitting a waiver request to change the hours program can commingle. LPA reviewed electronic sign in/sign out. LPA toured the inside and outside of the facility with Site Director. LPA observed that in Room 4 that there is cabinet under the sink that had cleaning supplies and a bottle of bleach. It has a lock, but was not locked. There were no children present during the inspection in that room. Site Director locked cabinet during inspection. LPA reminded Site Director that disinfectant or cleaning supplies need to be inaccessible to children. LPA also reminded Site Director to ensure that the door to the kitchen is closed at all times. Facility was observed to clean. Toys and equipment were age-appropriate and in good condition. There is a fully charged fire extinguisher, smoke detector, and carbon monoxide detector. The last fire/disaster drill was conducted on 06/29/2022. Site Director stated that there are no weapons, such as firearms, stored in the facility. -------------------------continues on 809 dated 07/27/2022 page 2---------------------------------- ----------------------continuation of 809 dated 07/27/2022 page 1---------------------------------- The outdoor activity areas is fenced. Area around play structure has resilient material. Shaded rest area is provided through trees. LPA observed that there were spider web on the play structure and on equipment. Director stated that they will deep clean the outside area. There were no bodies of water observed during today's inspection. Meals and snacks are prepared and brought from a third party. There is a kitchen to store and prepare meals and snacks. Drinking water is provided through water cooler and cups. Facility does not currently provide Incidental Medical Services (IMS). The records reviewed include but not limited to immunization and physician's report. C-1 and C-2 enrolled on 07/12/2022 and do not have physician's report on file. LPA discussed with Director that facility has 30 days to obtain to physician's report. The records reviewed include but not limited to transcript and Mandated Reporter training. S-1 last completed the mandated reporter training on 02/2022 and S-2 last completed the general mandated reporter training on 03/2019. S-3 was hired on 07/01/2022 and has not completed training yet. LPA reminded Site Director that training requires renewal every two years and to check that staff are completing the child care provider training. There is an staff on the infant program on site that has a valid CPR/1st Aid, which expires on 08/12/2023. Staff works from 7AM to 4PM. Director understands that there needs to be at least one staff with a valid CPR/1st Aid on site during hours of operation. All staff present have cleared fingerprints. Site Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. As a result of this inspection, three type B citations were issued. Exit interview conducted and report was reviewed with the Site Director, Perlina Chavez. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

    PERSONNEL REQUIREMENTS

    Based on record review, the licensee did not comply with the section cited above. None of the staff in the preschool program have a valid CPR/1st Aid. There is staff in the infant program on site that has a valid CPR/1st Aid, but works until 4PM, which posed a potential health, safety or personal rights risk to persons in care.

    Based on observation, the licensee did not comply with the section cited above. LPA observed that in Room 4 there is cabinet below the sink that had cleaning supplies and bottle of bleach that was not lock, which posed a potential health, safety or personal rights risk to persons in care. LPA also observed that the door to the kitchen was open, which has cleaning supplies inside of it.

    Based on record reviews, the licensee did not comply with the section cited above. S-1 and S-2 did not renew mandated reporter training from the last time they completed training, which poses a potential health, safety or personal rights risk to persons in care.
