On 09/25/2025, Licensing Program Analyst (LPA) Farida Raja arrived to conduct an unannounced Case Management inspection in response to an unusual incident that was self reported by the facility to Licensing on 09/17/2025. The incident occurred on 09/15/2025 involving children, C1 and C2. LPA toured the facility and interviewed Director and staff and obtained a copy of the children's roster. Based on interviews, it was determined that no staff observed any interactions between C1 and C2 in the classroom, outdoor area or bathroom that were of concern or resulted in C1 getting hurt. Based on available evidence at this time, no deficiencies were cited. Exit interview conducted and report was reviewed with Director, Anne Horgan. A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 DAYS.
