What the inspector wrote

Licensing Program Anal yst (LPA) Kate Huang conducted a Case Management inspection regarding an incident that was reported by the facility to Licensing. LPA met with Program Director, Oneiki Robinson. LPA interviewed staff and reviewed records and video footage. Based on information obtained from interviews and video footage, it was determined that on 10/20/2025 , between approximately 4:00 p.m. and 4:30 p.m. , one teacher (S1) pulled a child\342\200\231s (C1) hair, then C1 rubbed his head and walked away. One parent (P1) observed the entire incident. As a result of this inspection, one Type A deficiency was cited. LPA informed the program director that this report dated on 10/24/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed the program director to provide a copy of this licensing report dated on 10/24/2025 that document one Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted with the program director, Oneiki Robinson. A copy of this report was provided along with licensee's appeal rights. A Notice of Site Visit was issued and must remain posted for 30 days.
