What the inspector wrote

Licensing Program Analyst (LPA) Kate Huang conducted a Case Management inspection regarding an incident that was reported by the facility to Licensing. LPA met with Executive Director, Gladys Eng. LPA interviewed staff, one parent and reviewed a photo and video footage. Based on interviews, a photograph, and video footage, it was determined that on 02/03/2026, between approximately 11:20 a.m. and 11:23 a.m., one teacher (S1) roughly grabbed child (C1)\342\200\231s arms for about two minutes, causing red marks under the child\342\200\231s left armpit. As a result of this inspection, one Type A deficiency was cited. LPA informed the executive director that this report dated on 02/13/2026 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed the executive director to provide a copy of this licensing report dated on 02/13/2026 that document one Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted with the executive director, Gladys Eng. A copy of this report was provided along with licensee's appeal rights. A Notice of Site Visit was issued and must remain posted for 30 days.

The requirement was not met as evidenced by: based on interviews, a photograph, and video footage, it was determined that on 02/03/2026, between approximately 11:20 a.m. and 11:23 a.m., one teacher (S1) roughly grabbed child (C1)’s arms for about two minutes, causing red marks under the child’s left armpit, which posed an immediate risk to the health and safety of the children in care.
