Inspector’s narrative

What the inspector wrote

On May 7, 2026, Licensing Program Analyst (LPA) Marilou Monico met with Site Supervisor Rosemarie Catalan for an unannounced Case Management Inspection. The purpose of today’s inspection was to follow up on a self-reported incident submitted by the facility to the San Jose Regional Office on April 23, 2026, involving a child (C1) and a staff member (S1). The alleged incident stated that S1 pinched C1. An initial inspection was conducted on May 4, 2026. During today's inspection, LPA toured the facility, took a census, and conducted interviews with C1 and staff. During the investigation process, LPA interviewed staff and children, reviewed facility records, and obtained relevant documents. C1 stated that they were playing with peers when S1 pinched their hand. C1 demonstrated the action by gently holding LPA’s hand. Interviews with staff and other children revealed that no one observed S1 using inappropriate discipline or engaging in any behavior that would violate children’s personal rights. Based on the information obtained, no regulations were violated. Exit interview conducted and report was reviewed with Site Supervisor, Rosemarie Catalan. Appeal rights were provided. Per Title 22, Division 12, of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
