Licensing Program Analysts (LPA's) Anna Morales and Doni Fici conducted a Case Management inspection regarding an Unusual Incident that was reported by the facility to Licensing on 12/22/23. LPA's was greeted by Co-Site Director Elizabeth Huckaby. During the course of investigation, LPA's became aware that the former Director enrolled a child who was under two years old to attend the Preschool on 9/5/2023; The Center is licensed to serve ages 2-5 years old. LPA's toured the facility, interviewed staff, reviewed files, and obtained copies of documents. Based on interviews and evidence gathered, it was determined that on December 21, 2023, approximately at 10:15am, a preschool aged child was left unattended for approximately 5 to 7 minutes in the front parking lot (facing Murphy Ave). This poses an immediate risk to the health, safety, and personal rights to children in care. As a result of today's inspection, Two Type A citations were issued at today's visit. A copy of today's Facility Evaluation Report ( LIC809) has to be posted on the wall and a copy to be given to all parents of currently and newly enrolled children for the next 12 months. In addition, copy of LIC9224 Statement Acknowledging Receipt of Licensing Reports need to be signed and kept in child files. A citation warrants an immediate civil penalty of $500.00 and is hereby assessed, See LIC421IM. An exit interview was conducted, and Plan of Corrections were reviewed and developed with the Director. A notice of site visit has been issued and must remain posted for 30 days.
