What the inspector wrote

Licensing Program Analyst (LPA) Marilou Monico met with Dean of Community Education, Sam Bliss, and Director, Naoko Harada, and conducted a Case Management Inspection in response to an unusual incident report that was received by Community Care Licensing (CCL) on June 7, 2023. Naoko failed to report the alleged incident to CCL within the required time frame. The alleged incident is under investigation. As a result of this inspection, deficiency was cited on the following page. Exit interview conducted and report was reviewed with Dean of Community Education, Sam Bliss. A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 DAYS.

This requirement was not met as evidenced by: The Director failed to report an alleged incident to Community Care Licensing (CCL) within the required time frame. This poses a potential risk to the health, safety, and personal rights of children in care.
