What the inspector wrote

Licensing Program Analyst (LPA), Marilou Monico, made an unannounced Case Management Inspection. LPA met with Site Director, Tim Harper. On April 25, 2025, a report was received from the facility regarding incidents involving two children, C1 and C2. LPA toured the facility, interviewed staff, and obtained copy of Unusual Incident Report (LIC 624). Based on interviews, on April 23, 2025, a staff (S1) was on the patio area of Room 26 and observed a child (C1) knocking on the door of the classroom from inside. C1 was alone in the classroom. On April 24, 2025, a staff (S2) was in the sandbox area of the playground and witnessed a child (C2) inside Room 35 standing by the door. C2 was unattended in the classroom when their group transitioned outside. The parents of the two children were notified on 04/24/25 and Site Director met with them in person on 04/28/25. As a result of this inspection, Type A deficiency was cited on the following page. Assembly Bill (AB) 633 was provided and discussed with Site Director. LPA informed the Site Director to provide a copy of this licensing report dated April 30, 2025 that documents a Type A citation to parents/guardians of all children currently enrolled no later than the next business day or the next day the children are in care, and to parents/guardians of any newly enrolled children for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224) must be placed in the child's file for verification. Exit interview conducted and report was reviewed with Site Director, Tim Harper. A Notice of Site Visit was issued and must remain posted for 30 days.

This requirement was not met as evidenced by: On 04/23/25, a child (C1) was left unsupervised by staff in Room 26. On 04/24/25, a child (C2) was unattended in Room 35. This poses an immediate risk to the health, safety, and personal rights to persons in care.
