What Happened
Case management incident inspection following report of two children left unattended in classrooms. April 23: child found alone in classroom knocking on door. April 24: child found unattended in classroom when group transitioned outside. Type A deficiency cited for supervision violation.
Observations
- Child C1 on 04/23/2025 found alone and unattended in Room 26 classroom, knocking on door from inside
- Child C2 on 04/24/2025 found unattended inside Room 35 classroom while their group transitioned to outdoor play
- Parents notified on 04/24/2025 of incidents
- Site Director met with parents in person on 04/28/2025
