What Happened
Case management inspection to address supervision failure incident from October 15, 2025 where a child was found alone outside the Stage Yard playground gate. Type A citation issued requiring notification to all parents within next business day.
Observations
- Child found alone on 10/15/2025 walking between Room 35 and Staff Lounge building, outside Stage Yard playground gate
- Child initially in Stage Yard, staff in yard were unaware of unsupervised exit
- 4 staff members were in yard with approximately 25 children when incident occurred
- Parents were notified of incident
