What the inspector wrote

Licensing Program Analyst (LPA) Marilou Monico met with Site Director, Tim Harper, for an unannounced Case Management inspection regarding an incident that was reported to Licensing on December 5, 2025 involving a three-year-old child (C1) who sustained an injury at the center. LPA toured the facility and a census was taken. LPA interviewed staff and C1. Photos and ouch report were obtained. During the investigation process, LPA conducted observations, interviews, and obtained photos and ouch report. Based on the information gathered, on November 26, 2025, a child (C1) was playing on the wooden table with lid on the center section of the table. C1 removed the lid and the lid fell into the opening of the table and C1's hand got stuck between the table and the lid. Teacher (S1) rushed to help C1. C1 tried to pull their hand out and pinched the middle left finger. There were three staff members (S1, S2, and S3) with approximately 8 children present in Room 15 when the injury occurred. C1 was provided first aid. C1's parent was notified immediately. C1's parent took C1 to the doctor on the same day. The child sustained a fractured finger. C1 will use a splint on the middle finger for 6 weeks. After the incident, the table was removed from the classroom. Based on the information obtained, LPA determined that staff handled the incident correctly and took appropriate measures to address the child's injury. There were no deficiencies cited. Exit interview conducted and report was reviewed with Site Director, Tim Harper. A Notice of Site Visit was issued and must remain posted for 30 days.
