Inspector’s narrative

What the inspector wrote

On 03/13/2025 at 1:00pm, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu conducted an UNANNOUNCED COMPLAINT SITE INSPECTION. LPA met with Director Ananda Triulzi. LPA advised director of the nature of the inspection. A complaint regarding License was filed against the facility. LPA toured the facility for health and safety inspection. Present during the inspection were three (3) fingerprint-cleared staff and seven (7) children in care. LPA confirmed with director the Facility Roster and Personnel Report obtained on 01/10/2025 is still current as of today’s inspection. LPA conducted interviews with director and one (1) staff present at the facility. Interviews with director and staff both indicated a staff’s child under the age of two is occasionally present at the facility, which is out of the scope the facility’s license. Based on LPA’s observations and interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22 Division 12, 101161(a) is being cited on the attached LIC 9099D. Notice of Site Visit was given and must remain posted for 30 days. Report was reviewed, exit interview was conducted, and Appeal Rights were provided with Director Ananda Triulzi.
