What the inspector wrote

Licensing Program Analyst (LPA) Marilou Monico conducted a case management inspection regarding an unusual incident that was reported by the facility to Licensing on January 27, 2022. LPA toured the facility, interviewed staff, reviewed files, and obtained copies of documents. Based on interviews and evidence gathered, it was determined that on January 25, 2022, a 17-month-old child was found by a staff member unattended in the grass area outside the toddler yard. This poses an immediate risk to the health, safety, and personal rights to children in care. As a result of this inspection, deficiency was cited on next page. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS.

This requirement was not met as evidenced by: A child was found by a staff member unattended in the grass area outside the toddler yard on January 25, 2022. This poses an immediate risk to the health, safety, and personal rights to children in care.An immediate civil penalty of $500 was assessed.
