What the inspector wrote

Licensing Program Manager (LPM), Joel Segura, and Licensing Program Analyst (LPA), Marilou Monico, met with Site Director, Leslie Yamaki, and Regional Manager, Shelley Gonzales, at the San Jose Regional Office today for a scheduled Informal Meeting . The purpose of today's meeting is to discuss the following issue: This informal conference is a direct result of recent type A citation: 02/02/22 - Section 101229(a)(1)- Care and Supervision - A child was found by a staff member unattended in the grass area outside the toddler yard on January 25, 2022. Civil penalty of $500.00 was assessed. The facility implemented the following plan to bring the facility in compliance: 1) Check gates and doors to ensure they are closed 2) Teachers need to spread out on the playground to observe the children 3) Frequent headcounts 4) Staff training on supervision of children was conducted on February 1, 2022. Site Director and Regional Manager agreed to increase supervision. Site Director and Regional Manager agreed to provide ongoing staff training on care and supervision, head counting, and positioning of staff. Training agenda and proof of attendance shall be maintained at facility for Community Care Licensing review. Site Director and Regional Manager agreed to conduct daily monitoring to assure staff are properly positioned and providing adequate supervision. LPM explained to Site Director and Regional Manager that if there are continued deficiencies cited for the issue noted on this report, the Facility may be referred to legal department for possible administrative action, which could include revocation of the Facility license. The Facility will be monitored for the next 12 months to ensure that the Facility is in compliance with the Department regulations. Assembly Bill 633 (Child Care Parent Notification Requirements) and Acknowledgement of Receipt of Licensing Report (LIC9224) was also explained and provided to Site Director and Regional Manager during today\342\200\231s meeting.
