On 08/28/2025 at 8:15am, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu met with Director Mayra Howard for an unannounced case management inspection for the purpose of discussing LPA’s observations at the facility on 08/14/2025 when the Director was not on site. Present during the inspection today were the director, six (6) fingerprint-cleared staff, and 19 infants in care. The facility is within ratio and in compliance with capacity regulations today. The facility was toured to conduct a Health and Safety Inspection. On 08/14/2025, LPA conducted an inspection on site. During that visit, there were 25 infants in care. Director Mayra Howard was not on site during the inspection due to illness. Designated facility representative Kufre Udoeyop was also not on site during the inspection due to illness. Instead, designated facility representative Kimberly Helms was on site. However, the facility did not have an assistant director on site. As of today, the facility does not have a second director position or assistant director position. Per Licensing regulation, in addition to the director, an assistant director shall be present and on duty if the center has 25 or more infants in attendance. (Type B citation issued.
