What the inspector wrote

On 06/03/2026 at 8:25am, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu conducted an unannounced case management inspection for the purpose of investigating a reported incident. LPA met with Assistant Director Gwendolyn “Gwen” Page. Also present during the inspection were five (5) fingerprint-cleared staff and 15 infants in care. The facility is within ratio and in compliance with capacity regulations today. The incident occurred on 05/27/2026 and was self-reported to Licensing on 05/28/2026. The incident was regarding an interaction between a staff member (S1) and a child (C1). After the witness reported the incident to Director, an internal investigation was conducted. During today’s inspection, LPA reviewed the above information with Director and conducted interview to obtain more information. Per Assistant Director, the internal investigation is still ongoing. The incident occurred in the outdoor play area. Interviews were conducted with the witness, S1, and staff that were present during the incident. Based on the information that was available during today’s inspection, C1’s Personal Rights were determined to not be violated at this time. Assistant Director and Director will provide more information regarding the incident once it is available. The determination for the reported incident may change based on that information, and LPA may return for another inspection for this incident. No deficiencies were issued during today’s inspection. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to Director Gwen Page. Page 1 of 1.
