Inspector’s narrative

What the inspector wrote

Based on observation, and Interviews conducted with staff and parents. LPA was informed that all staff who were interviewed stated they change gloves at every diaper change when caring for children. Staff mentioned to LPA their procedures on diaper changing and how diaper changing table surfaces are cleaned, sanitized, and wiped down. LPA was informed about their procedure on incident reports and process of composing them; staff stated director will be informed about the incident, along with parents of the child and an incident report is generated. Parent's who were interviewed informed LPA that staff have changed gloves prior to diaper changing of children and have cleaned and wiped down diaper changing tables before the next child uses it. LPA was also informed by 3 parents who were interviewed stated they have personally witnessed staff practice good hand hygiene during diaper changes. Based on LPA’s observation on unannounced visits, on 2/20/2025, and 4/11/2025, Infants and toddlers classroom diaper changing areas were observed cleaned, sanitized, wiped down, and free from stains and feces before another child is placed on the changing table. LPA observed cleaning supplies and gloves available to staff. LPA also observed staff wearing gloves when changing children, washing their hands after diaper changing, and observed staff cleaning and sanitizing the diaper changing station. It was alleged that, Staff do not keep the child's authorized person informed about the child's care. Based on interviews conducted with staff and parents, LPA was informed that parents are told about any incident report that occur in the center with their child; Staff mentioned incident reports are written by staff and another teacher will review the incident report to ensure the clarify of what is being written. Staff 1 (S1) informed LPA when an incident report is written by staff, staff will let the Director review the incident report before the Director approves it; Staff will also inform parents about the incident. Parents that were interviewed informed LPA that staff and director do inform parents about incidents that occur at the center with their child; parents expressed no concerns regarding staff and director not informing parents about incidents. Based on interviews, and observations, and evidence gathered during the course of the investigation, it is concluded that although the allegations noted on this complaint may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. The allegations are UNSUBSTANTIATED . A notice of site visit has been issued and must remain posted for 30 days. Exit interview conducted with director, and report was reviewed and provided.
