What the inspector wrote

Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Case Management- Incident. LPA met with Director Maarit McCrossen and explained the reason for the inspection. The purpose of this inspection is in response to an incident that was self-reported by the facility to Licensing on 10/06/2022 involving potential violation of personal rights, which occurred around the beginning of 10/2022. Present during today's inspection were 57 children and at least 6 staff. During today's inspection, LPA conducted observation and reviewed staff files. LPA also interviewed staff and children. Director stated that they conducted their own investigation. Director will send notes from their investigation to Licensing by 5PM on 11/10/2022. Based on the information obtained, S-1 and S-2 did violate chi ldren's personal rights. S-1 and S-2 are no longer working at the facility. Director stated that she conducted training on personal rights on 10/10/2022 with staff. Director will send notes on training to Licensing. LPA also observed around 11AM that the preschool children were using the school-age yard. There were no school-age children present at the time. LPA discussed with Director that preschool children cannot use yards that are licensed for other program. --------------------continues on 809 dated 11/09/2022 page 2---------------------- ------------------continuation of 809 dated 11/09/2022 page 1--------------------- LPA Samantha Yip informed Director Maarit McCrossen that this report dated 11/09/2022 documents two Type A citations which shall be posted for 30 consecutive days as there /are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Samantha informed the Director, Maarit, to provide a copy of this licensing report dated 11/09/2022 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. As a result of this inspection, two Type A citations were issued. Exit interview was conducted and report was reviewed with Director Maarit McCrossen. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
