What the inspector wrote

On 8/23/2024, at 12:05 PM, Licensing program analyst (LPA), Doni Fici arrived unannounced to conduct a Case Management investigation. LPA was greeted by Director, Maarit McCrossen and explained the purpose of today's visit. During the investigation, LPA interview three (3) staff. Based on interviews conducted with staff, three (3) out of 3 staff stated staff thought Child 1 (C1) was checked out at the time of the incident. Staff 3 (S3) did a roll call and counted 11 children, and staff 2 (S2) conducted a roll call and counted 10 children. S3 did a visual sweep of the yard and did not see C1. S3 brought all the children into the class while being transitioned inside and C1 was not seen in the class at 5:26PM. At 5:27PM, S2 went outside and did a sweep of the yard and saw C1 walking towards the class alone; C1 was seen by the slide of the playground. Parents were informed and notified. Director stated an emergency staff meeting was conducted on 8/20/2024 for a plan of action to immediately retain staff in CSR (Child Supervision Record),transitions, and Kinder Care app. Director stated she took disciplinary action to resolve the matter on the same day of retaining staff. A civil penalty of $500 is being assessed during today\342\200\231s investigation. The following type A deficiency was cited on the attached page (809-D). Licensee was informed that failure to correct the deficiency by the specified Plan of Correction (POC) Due Date may result in assessment of civil penalties in the amount of $100 per day per violation until the correction is made. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview conducted with Director, and a copy of this report review and provided along with appeal rights.

(1) No child(ren) shall be left without the supervision of a teacher at any time...This requirement is not met as evidenced by: Based on interviews, the licensee did not comply with the section cited above by not supervising C1 appropriately and leaving C1 unsupervised without an adult present which poses a potential health, safety or personal rights risk to persons in care.
