Inspector’s narrative

What the inspector wrote

It was alleged that, Absence of supervision of a child. Based on interviews conducted with staff, LPA was informed that Child 1 (C1) was visually supervised by staff while C1 was standing in the 2\342\200\231s B-classroom by the gate while the teachers on duty were either changing children\342\200\231s diapers or transitioning children outside. LPA was informed that the staff were visually supervising C1 while he was standing in the B\342\200\231s classroom by the gate. It was alleged that, Staff do not keep the childcare clean and sanitary, and staff are using the same disposable gloves to change the diapers of multiple children. Based on observation and interviews conducted with staff, LPA was informed that staff clean and sanitize the children\342\200\231s diaper changing station after every diaper change; LPA also was informed that staff always wash and wear new gloves prior to every diaper change. Based on LPA\342\200\231s observation on unannounced visits, on 2/20/2025, 4/11/2025, and 4/17/2025, preschool 2's classrooms diaper changing station were observed cleaned, sanitized, wiped down, and free from stains and feces before another child is placed on the changing table. LPA observed cleaning supplies and gloves available to staff. It was alleged that, Staff do not keep the child's authorized person informed about the child's care. Based on interviews conducted with staff, LPA was informed that parents are told about any incident report that occur in the center with their child; Staff mentioned incident reports are written by staff and another teacher will review the incident report to ensure the clarify of what is being written. Staff 1 (S1) informed LPA when an incident report is written by staff, and staff will let the Director review the incident report before the Director approves it; Staff will also inform parents about the incident. Based on interviews, observation, and evidence gathered during the course of the investigation, it is concluded that although the allegations noted on this complaint may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. The allegations are UNSUBSTANTIATED . A Notice of Site Visit was given and must remain posted for 30 days . Exit interview conducted with assistant director, and a copy of this report review and provided.
