Licensing Program Analysts (LPAs) Jessica Bongardt and Mel Matos conducted an unannounced case management inspection. LPAs met with Rehka Mundkur, Director, and explained the reason for today's inspection. Purpose of today's inspection: Facility is applying to add Room 15 to the existing license (capacity remains unchanged). The Facility is currently licensed in Rooms 8, 12, 13, 14, Study Room, and Library. A fire clearance approval from Santa Clara Country Fire was received on 11/15/2023. Hours of operation are Monday through Friday 8:15 AM to 4:00PM. LPAs observed the required postings at the front entrance of the Facility and in each Room. LPAs toured the entire Facility and measured Room 15 during today's inspection. LPAs observed 7 toddlers in room 8 with 3 teachers, 15 preschool children in room 12 with 4 teachers, 17 preschool children in room 13 with 2 teachers, and 13 preschool children in room 14 with 3 teachers during today's inspection. Measurements of Rooms 8, 12, 13, 14, Study Room, and Library and outdoor space were taken by LPA Zaid Hakim on August 14, 2019. Outdoor Measurements: Rear Outdoor Play Yard 56.417 x 32 = 1,805.344 + 100 x 20.583 = 2,058.3 = 3,863.644 SQ FT Toddler / Classroom 8 Outdoor Play Yard 18.500 x 12.750 = 235.875 + 14 x 15 = 210 + 25.583 x 17.917 = 458.370 = 904.245 SQ FT Front Outdoor Play Yard 47 x 35.833 = 1,684.151 + 44.667 x 42.500 = 1,898.347 + 36 x 25.667 = 924.012 = 4,506.51 SQ FT Total Outdoor Space measurements: 3,863.644 SQ FT + 904.245 SQ FT + 4,506.51 SQ FT = 9,274.399 TOTAL OUTDOOR SQ FT divided by 75 = 123 children. The Facility is clean, safe, sanitary, and in good repair. The Facility has adequate lighting and heating/air conditioning in all Rooms. There are no wall heaters or fireplace units in the Facility. Children bring their lunches from home and the facility only provides AM and PM snacks to the children. Disposable utensils and plates will be utilized by the Facility. LPAs observed solid waste containers with a tight-fitting lids in all Rooms and in the playground areas. Drinking water inside and outside will be provided through reusable water bottles and water dispensers. LPAs observed enough chairs, tables, cubbies, mats, and hooks for all children at the facility. There is only cold running water in the children's sinks. LPAs advised Rehka to keep cleaning supplies inaccessible to children in higher cupboards or securely locked lower cupboards. There are no poisons on the premises. There are enough materials, toys, and supplies for the children in care. All equipment and furniture are safe and age appropriate. There are sufficient toilets and sinks for the preschool children within the Facility. Staff have separate staff bathrooms on site (located in the main church building) that can be utilized by sick children if necessary. Rekha states that the Facility is considering adding a urinal to the boy's bathroom to allow the Facility to allocate a bathroom stall for "staff use" since the Facility is considering have the staff bathroom closer to the children's bathrooms. LPA's advised Rekha that the Department will need to be notified if the Facility elects to add a urinal to the boy's bathroom for a follow up inspection. Sick children will be isolated in the front office area of the Facility and isolation equipment is available. There is a fully charged fire extinguisher, smoke detectors, and carbon monoxide detectors. LPAs observed a complete first aid kit in each Room. Rehka states that there are no weapons/ammunition on the premises. The preschool and toddler playground areas are adequately fenced. There is sufficient equipment, toys, and materials for the preschool and toddler children outdoors. LPAs discussed supervision requirements of the children whenever they are in the playground areas. Shaded rest areas are available throughout the playground via building overhangs, trees, and canopy. There were no bodies of water observed during today's inspection. Areas around the age appropriate & secure play structure and slide have resilient material (rubber surfacing and wood chips). Transportation will not be provided by the Facility. The Facility has a working telephone: 408-356-8816. Rehka Mundkur, Director, was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Exit interview conducted and report was reviewed with Rehka Mundkur, Director. No deficiencies issued during today's inspection. Notice of site visit was issued and must remain posted for 30 days. LPAs advised Rehka Mundkur that the additional room (Room 15) will be approved and added to the License upon completion of the following: 1. Final review and approval by Licensing Program Analyst's manager.
