What the inspector wrote

At 8:30 AM, Licensing Program Analyst (LPA) Andy Yang, met with Assistant Site Director, Tiffany Bui, for an unannounced case management investigation. Purpose of today's investigation: discuss an incident report that the Facility self-reported to the Department on April 28, 2025, due to a child receiving an injury while in playground. LPA toured the indoor area and outdoor of the facility . During today's investigations, LPA interviewed with the Assistant Site Director and staff, and reviewed reports and a child\342\200\231s file. Based on inspection, interviews, observation conducted, and review of the video, a child was walking alongside the outside playground area, and due to inconclusive evidence that the child had injured himself in the playground from the fence which did not pose an immediate risk to the child. LPA observed the opening in the fence where the child was walking along. LPA reminded that outdoor activity space shall maintained in a safe condition for activities planned. Facility provided a work order to fix the fence. No deficiencies were issued during today's investigation. (1) Technical Violation was issued during today\342\200\231s inspection. Please see attached advisory note pages for information on technical violations issued today. Note that technical violations are not citations but are advisory notes. Technical Violation \302\267 Facility to fix the fence, the gap between the metal fence and the wooden fence. A notice of site visit and appeal rights were given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Assistant Site Director, Tiffany Bui.
