What the inspector wrote

Licensing Program Analyst (LPA) Samantha Yip conducted an announced Case Management- Licensee Initiated inspection. Due to COVID-19 and shelter in place, a tele-inspection was conducted via Facetime. LPA met with Site Director Oksana Oks and explained the reason for the inspection. The purpose of this inspection is facility originally requested to change capacity to 92 and add a toddler option. Facility withdrew request for toddler option and has r equested a capacity of 96 in Room 1, Room 2, Room 3, and Room 5 for ages 2 to 5. There is also a infant and school-age program on site. LPA informed Site Director that a copy of this report will be emailed to her. Site Director's response to email will serve as acknowledgement that report was received. The hours of operation are Monday through Friday 7AM to 6PM. The State Preschool's hours of operation are Monday through Friday 830AM to 11:30PM. The state preschool is currently not in session due COVID. An fire clearance for 96 children was granted on 03/19/2021. Site Director guided LPA on a tour of the facility via Facetime. There has been an update to the Room 3. Measurement for Room 3 were conducted during today's inspection by Site Director Oksana Oks to reflect the changes. Indoor and outdoor measurement for Room 1, 2, 5, Preschool Yard, and SA Yard/Preschool were conducted on 10/02/2019 and 12/11/2019 by LPA Yip and LPA Rodriguez The measurements are as followed: Room 1 : 29.250 x 27.250 + 21.417 x 6.417 = 941.807 minus encumbered space 1.750 x 1. 833 +2.583 x 1.667 + 2.167 x 4.083 + 1x13= 29.359 = 912.448 -----------------CONTINUES ON 809 DATED 04/08/2021 PAGE 2---------------------------------- ----------------------continuation of 809 dated 04/08/2021 page 1---------------------------------- Room 2: 12.583 x 6 + 5.583 x 16.167 + 24.167 x 37.583 = 1074.026 minus encumbered space 8.333 x 1.250 + 11 x 3 + 1.083 x 2 + 4 x 2.167 + 1.750 x 1.833 + 1.333 x 2 = 60.123 = 1013.903 Room 3: 24.333 x 37.583 + 12.750 x 5.833 + 3 x 2.750 + 2.083 x 6.500 + 2 x 13 + 2 x 7.583 = 1051.832 minus encumbered space 2.417x 1.917 + 6x2 + 1.750x1.667 + 3.833x2.167 + 3.667x2.083 + 2(1.250x4.250) = 46.119 = 1005.711 Room 5: 35 x 29.333 = 1026.655 + 9.667 x 2 = 19.334 + 7.917 x 2.167 = 17.156 + 2.917 x 8 = 23.336 = 1086.481 minus encumbered space 8 x 1.250 = 10 = 1076.481 TOTAL INDOOR SPACE MEASUREMENT = 912.448 + 1013.90 + 1005.711 + 1076.481 = 4008.543 divided by 35 = 114 children The outdoor space measurements are as followed: SA Yard/Preschool: 20.333 x 2 + 9 x 27 + 37.833 x 25 + 17 x 7 + 13.917 x 5 + 32.333 x 15.333 minus encumbered space 1.500 x 1.667 = 2.500 = 1911.336 divided by 75= 25 children Preschool Yard: 17.583 x 19.250 + 14 x 21.833 + 36.167 x 25 + 17 x 12.250 + 55 x 25.417 + 21.500 x 26.583 = 3726.028 minus encumbered space 2(1.583 x 1.500) = 4.749 = 3721.279 divided by 75 = 49 children Total Outdoor Space= 25 + 49 = 74 children LPA observed that there are nine (9) toilets, one (1) urinal, and fourteen (14) sinks for children' use and three (3) additional sinks in the yard for children to use. There is sufficient amount of chairs, tables, and napping mats/cots for the requested number of children. There is sufficient amount of age appropriate toys for children in care. There is a changing table in Room 3. Storage containers for waste have tight fitted lids. Restroom for children was observed to sanitary. Site Director stated that there are no weapons, such as firearms stored on the premise. There a fire extinguisher, smoke detector, and carbon monoxide detector. ----------------------CONTINUES ON 809 DATED 04/08/2021 PAGE 3---------------------------- --------------CONTINUATION OF 809 DATED 04/08/2021 PAGE 2--------------------------- The outdoor area is fenced. Shaded rest areas are provided through the canopy, building overhangs, and pergola. Facility does have waiver to share outdoor space with the school-age program. There were no bodies of water observed during today's inspection. Facility does provide meals and snacks to children. LPA informed Site Director that an updated license reflecting the change of capacity will be issued upon approval of Licensing Manager and completion of the following items: - waiver request for outdoor square footage and outdoor usage schedule by 04/16/2021 No deficiencies were cited as a result of this inspection. An exit interview was discussed, where this report was discussed and emailed to Site Director, Oksana Oks. A Notice of Site Visit has been issued and must be posted for 30 consecutive days.
