What the inspector wrote

On 5/27/2026, Licensing Program Analyst (LPA) Liridon Fici (Doni) conducted an unannounced Annual Inspection and met with Director, OKS, OKSANA and informed her the purpose of today’s visit. LPA gathered a census during visit. LPA also reminded Director to make sure all Annual Fee’s are paid. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Child Car Seat Law (PUB 269), Menus, and Activity Schedule. Days and hours of operation are Monday through Friday 7:00AM to 6:00PM. LPA did not observe any bodies of water during inspection. LPA observed that the teacher/child ratio was in compliance during today's inspection. LPA observed 19 children with three (3) teachers in preschool 1, 18 children with two (2) teachers in preschool 2, and 13 children with 3 teachers and one (1) aide in early preschool class. Director understands the conditions, limitations, and capacity specifications of the Facility license. She understands that children should be always visually supervised. Any child(ren) who exhibit symptoms of illness including, but not limited to, fever or vomiting, are not accepted into care. Any child(ren) who become ill during the day shall be isolated in the Director's office until the child is picked up by parent(s). LPA reviewed six (6) children's files and observed that parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. This annual will continue at a later date. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS. Exit interview conducted and report was reviewed with Director, OKS, OKSANA along with appeal rights provided.
