What the inspector wrote

On 4/16/2026, Licensing Program Analyst (LPA) Liridon Fici- Doni conducted an unannounced case management visit in response to an incident report that the department received on March 30, 2026. LPA met with Director, Martha Morales and informed her the purpose of today's visit. LPA also gathered a census of children. During visit, LPA interviewed the director and obtained documents on training that was conducted with staff. On March 30, 2026, the Director reported that during a diaper-changing routine, Child #1 (C1) entered the classroom bathroom without informing staff of the need to use the restroom. The Director stated there is no changing table located inside the bathroom. At the time of the incident, Staff #2 (S2) was in the bathroom changing another child’s diaper while facing the child. Staff #3 (S3) was seated across from the bathroom area, assisting other children with an individualized activity. The Director stated that C1 entered the bathroom without notifying staff. After S2 completed the diaper change and attempted to close the bathroom door, S3, who was walking past the bathroom, heard a scream. S3 immediately grabbed the doorknob, opened the door, and observed C1 sitting on the far-right toilet. The Director reported that there were three (3) staff present on March 30, 2026, that being one associate teacher, one lead teacher, and one substitute teacher, supervising a total of 15 children. The Director stated that, following the incident, staff training was conducted on March 30, 2026, and March 31, 2026, after all children had left. Page 1 of 2. The training included active supervision practices, child health and safety, a video from the Department of Child Care Licensing on active supervision, review of headcount procedures during major transitions, and Head Start supervision procedures. The Director further stated that a face-to-name bathroom chart has been implemented to track which children are using the restroom to prevent similar incidents. Additionally, the Director reported that a follow-up staff training is scheduled for April 30, 2026, which will cover licensing personal rights, toileting procedures, and active supervision and will end the same day; LPA asked the director to submit proof of follow-up training with staff to LPA after training is complete. The following type B deficiency was cited on the attached page (809-D). Licensee was informed that failure to correct the deficiency by the specified Plan of Correction (POC) Due Date may result in assessment of civil penalties in the amount of $100 per day per violation until the correction is made. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and this report was reviewed with the Director, Martha Morales along with appeal rights provided.
