What the inspector wrote

On June 16, 2025 at 8:51 AM, Licensing Program Analyst (LPA), Marilou Monico, conducted an unannounced Annual Random Inspection. LPA met with Licensee, Bhavani Mummidivarapu, and explained to her the purpose of today's visit. Also present in the home were licensee's husband, licensee's father-in-law, and licensee's adult daughter. Licensee received the Entrance Checklist - Family Child Care Homes (LIC 126). LPA observed all required posting materials. The daycare is open Monday thru Friday from 8:30 AM to 6:00 PM. There are no active waivers or exceptions for this facility. Licensee's adult helpers (S1 & S2) and 10 children arrived after 9:00 AM. Licensee stated that there are three (3) adults residing in the home: herself, her husband, and her father-in-law. Fire drill was conducted on April 23, 2025. Licensee was reminded that fire/disaster drills must be conducted and documented every six months. LPA observed a fully charged 3A40BC fire extinguisher, glass covered fireplace, working smoke and carbon monoxide detector. Cleaning products and sharp objects were stored inaccessible to children. LPA observed sufficient age-appropriate materials, toys, and play equipment in the home. The children's bathroom is clean, sanitary, and operable. Off limit areas inside the home: 4 bedrooms, 3 bathrooms, living room, and kitchen. Off limit areas outside: detached garage and right side yard. LPA advised licensee to notify Licensing for any change from an area of the family child care home previously identified as "off limits" to an area where care and supervision will be provided to children in care. The Licensee was reminded to notify the department prior to any alterations or additions to the home or grounds. Licensee confirmed that there are no weapons or firearms on the premises. There were no bodies of water observed. The home has a working telephone 408-838-6600. Continuation on next pages: This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm Licensee, Bhavani Mummidivarapu, reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Ten children's files were reviewed during today's inspection for the following records: Notification of Parents Rights (LIC995A), Consent for Emergency Medical Treatment (LIC627), Affidavit Regarding Liability Insurance (LIC 282), Identification and Emergency Information (LIC700), and Immunization Records. LPA reviewed three assistants files (S1, S2, & S3). Licensee has immunizations in measles, pertussis and flu. Licensee has current Mandated Reporter Training expiring on June 4, 2026. Licensee has current CPR/First Aid certification with an expiration date of July 10, 2026. Licensee was reminded that Mandated Reporter Training must be renewed by all staff every 2 years. LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also reminded Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee, Bhavani Mummidivarapu, was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&R's) throughout California. As a result of this inspection, there were no deficiencies cited. Exit interview conducted and report was reviewed with Licensee, Bhavani Mummidivarapu. During the exit interview, the Licensee, Bhavani Mummidivarapu, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS.
