What the inspector wrote

On 12/09/2025, Licensing Program Analyst (LPA) Jennifer "Jen" Beehler conducted an unannounced Case Management - Other inspection. LPA was greeted by the Director and provided access to the facility. Based on conversations with the Director, it was determined the facility would like to license additional space for the program that is utilized on occasion by the children in care. Director stated the gymnasium on campus was used on occasion for play time when rain prohibited the children from playing outside. Director stated the church sanctuary was utilized monthly for a joint assembly and classroom #10 was utilized as the art lab regularly for children to complete art projects. LPA toured the spaces with the Director and observed the spaces to be safe for preschool age children. LPA observed the equipment children would use, such as tables and chairs were in good condition and all hazardous materials such as cleaning solutions were inaccessible to young children. LPA discussed the Licensee's information on file with the Department and requested updated documents be submitted to the Department to reflect the Licensee's current Board and authorized signers for the organization. The Director stated she would work with the licensee's Board to collect the required documentation to update the Licensee records with the Department. LPA encouraged the Director to review all documentation for the facility and staff to ensure it is current and up to date. Continued on Page 2 LPA requested the Director to provide the following updated facility documents by 12/19/2025: 1. LIC999 - Facility sketch for each additional space and an updated Bird's Eye Sketch showing the campus and what buildings are utilized by the program. 2. LIC610 - Emergency Disaster Plan incorporating the new spaces. 3. LIC308 - Responsible Designee 4. LIC309 - Administrative Information LPA will review these documents and request a fire clearance inspection when the documents are received and complete. During today's visit, LPA collected an updated LIC200A with updated program information. LPA provided blank copies of required documentation to the Director. Due to today's inspection no deficiencies are cited. Exit interview conducted with the Director. Report was reviewed and provided along with appeal rights. A NOTICE OF SITE VISIT WAS PROVIDED AND MUST REMAIN POSTED FOR 30 DAYS.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    101229 Responsibilty for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement has not been met as evidenced by: LPA observed four out of the five classrooms without a Fully Qualified Teacher present. This poses a potential risk to the health, safety and personal right of children in care.

    HSC 1596.861 License number; use in advertisements; publications, or announcements (a) Each child day care facility licensed under this chapter, Chapter 3.5 (commencing with Section 1596.90), or Chapter 3.6 (commencing with Section 1597.30) shall reveal its license number in all advertisements, publications, or announcements made with the intent to attract clients. This requirement has not been met as evidenced by: Facility website did not have facility license posted. This poses a potential risk to the health, safety and personal rights of children in care.

    1596.8662 ....Training for mandated reporter who is licensed day care provider, administrator, or employee of a licensed child day care facility (b)(3) On and after January 1, 2018, a person who becomes an administrator or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a) within the first 90 days that he or she is employed at the facility and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement has not been met as evidenced by seven out of 12 staff members did not have valid Mandated Reporter training on file. This poses a potential risk to the health, safety and personal rights of children in care.
