What the inspector wrote

On 01/09/2026 Licensing Program Analyst (LPA), Jaime Gonzales was greeted and granted access to the facility by Site Director, Marija Palihnic Fisher. LPA stated the reason for todays visit was to conduct an unnanounced annual inspection. LPA took a census of the classrooms in rooms A, B, C and Early Montessori. LPA observed classroom A and B outside combined with 32 preschoolers and four staff, classroom C with 11 preschoolers and one staff, and early Montessori (toddler) classroom with 10 toddlers and 3 staff. LPA toured both indoors and outdoors of the facility during the inspection. LPA observed all required postings in all classrooms. Hours of operation for the facility are Monday – Friday, 7:30AM-600PM. The classrooms have age-appropriate materials, toys, furnishing, equipment, & adequate storage for children’s belongings. The children's napping equipment were observed to be stored in a healthful manner. Bedding is provided by the parents and is washed weekly or as needed. LPA observed two fully charged 3A40BC fire extinguishers, functioning smoke and carbon monoxide detectors. The isolation area for sick children is next to the teachers desk. Facility offers (full) day program and provides midmorning and afternoon snack to children. Snacks are stored in the classroom. The center provides hot lunch provided by a private catering company. Parents also have the option of bringing in meals. Menu is in writing is posted and accessible to authorized representatives. The kitchen and storage area is clean and free of litter and rubbish. Equipment necessary for the storage, preparation and service of food is well maintained, clean, and sanitized after each use. All center provided food items are properly labeled & stored separately from cleaning supplies. Continued on page 2... Drinking water is readily available for children in the facility via water bottles from home labeled with children's names. Water is refilled throughout the day by teachers and each classroom has filtered water systems. Trash cans for the disposable of solid wastes are equipped with tight fitting lids. The outdoor play areas are fully fenced. Shade is provided by trees and canopies. There are no bodies of water on the premises. There are age-appropriate outdoor toys and materials for the children. LPA observed climbing structures with sufficient resilient material under and around the structure. At least one staff member has a current Pediatric CPR/First-Aid certification. Facility utilizes an electronic application for authorized representatives to sign-in/out called Lillio. Facility conducts a Fire/disaster drill every month and the last drill was conducted on November 18, 2025. LPA obtained a copy of the facility roster (LIC9040) during today's inspection. LPA reviewed 8 random children files during today's inspection for the following records: Admission Agreement, Personal Rights (LIC 613A), Identification and Emergency Information (LIC 700), Notification of Parents Rights (LIC 995A), Consent for Emergency Medical Treatment (LIC 627), Physician’s Report with TB test (LIC 701), Immunization Records and 15 minute infant sleep logs. LPA reviewed staff files for the following records: Health Screening Report (LIC 503), TB Test, Statement Acknowledging Requirement to Report Child Abuse (LIC 9108), Employee rights (LIC 9052), staff qualifications, and required training. All staff have Mandated Reporter Training. Site Director was reminded of their responsibility to renew CPR/First Aid and Mandated Reporter Training every two years. During record review LPA observed Staff (S1, S2, S3, S4) were missing vaccinations records for measles and pertussis. In addition, LPA observed Staff (S2 and S3) were missing criminal record clearance associations to Canyon Heights Academy and (S4) does not have a current criminal record clearance through the Department of Social Services. Site Director, Marija Palihnic Fisher was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with Site Director, Marija Palihnic Fisher and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed SSite Director, Marija Palihnic Fisher of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. LPA reviewed with Site Director LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. On this date, 01/05/2026, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Continued on page 4... Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Site Director, Marija Palihnic Fisher was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Site Director, Marija Palihnic Fisher confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. The following type A and B deficiencies were cited on the attached pages (809-D). Site Director, Marija Palihnic was informed that failure to correct the deficiency by the specified Plan of Correction (POC) Due Date may result in assessment of civil penalties in the amount of $100 per day per violation until the correction is made. LPA, Jaime Gonzales informed Site Director, Marija Palihnic that this report dated 01/09/2026 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Jaime Gonzales informed the Site Director, Marija Palihnicto provide a copy of this licensing report dated that 01/09/2026 documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal Rights Provided. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Site Director, Marija Palihnic Fisher and Principal, Analiza Filion.
