What the inspector wrote

On 11/21/2025 at 1:30 pm, Licensing Program Analyst’s (LPAs) Manel Estoesta and Paulita De La Cruz conducted a Case Management Visit due to an Incident Report. LPAs met with Director Christine Nazario and explained the nature of the visit. On this visit there were 27 Staff Members and 77 children (infants, toddlers and preschool). On 11/10/2025 rodent droppings were discovered by a teacher in Mercury and Viking classrooms. On 11/12/2025 rodent was discovered in Apollo and Vanguard classrooms by a teacher. On the above dates, parents in the respective classrooms were notified, and children were sent home. From the 11/12/2025 to 11/182025, steps were taken, such as, deployment of Hazardous Waste Team, Hazardous Waste janitors to clean and sanitize all the classrooms. On 11/19/2025, the 4 classrooms mentioned above, resumed operations. LPAs conducted record reviews, staff interviews, and health and safety inspections. Based on the record review and staff interviews, the Licensee was in violation of Section 1012381 - Buildings and Grounds – (a) the child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors (1) the licensee shall take measures to keep the center free of flies, other insects, and rodents. LPAs informed the Director Christine Nazario that this report dated 11/21/2025 included a Type B Citation which can be posted as there is a potential risk to the health, safety, or personal rights of children in care. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed by Director Christine Nazario.
